Define your community.

  1. Prepare your survey questions
    Considering your audience and your issue, craft questions that will inform you on the 5 key indicators of community readiness:

    Examples of communities include:
    1. Community Knowledge of Efforts – How much does the community know about the current programs or efforts?
    2. Leadership – What is leadership’s attitude toward addressing the issue?
    3. Community Climate – What is the community’s attitude toward addressing the issue?
    4. Community Knowledge of the Issue – How much does the community know about the issue?
    5. Resources – What are the resources that are being used or could be used to address the issue?

Consider also adding demographic questions that can better inform you on your audience for the action plan. Questions might ask about their age, ethnicity, gender, job title, marital status, etc.
